REQUEST TO CANCEL RETAIL SALES CONTRACT

NOTICE TO BUYER:
 
		
	ASF International, Inc. is a financial services company located in Denver,
	Colorado.  You have been making installment payments to us due under the
	terms of the Retail Sales Contract you have signed.

	We provide a credit line to facilities.  In return, facilities assign all
	rights to future payments due under all Retail Sales Contracts to us. Therefore,
	all authority to cancel, alter or modify your Retail Sales Contract rests solely
	with us.  The facility has no authority to cancel your Contract, alter the total
	amount due, or modify the payment schedule in any way whatsoever.

	The terms of some Retail Sales Contracts provide for cancellation under certain
	circumstances; for example, if you have permanently moved your residence more
	than 25 miles from the location of the facility and a comparable facility is
	not provided, if you have a proven medical condition that permanently prevents
	physical participation, and at times, for other reasons stated explicitly in your
	Retail Sales Contract.

	If you believe you have a valid reason to cancel your Retail Sales Contract,
	you must fill out the Application To Cancel Retail Sales Contract and send it to
	us.  We will contact you in writing within ten days of receipt of your application.
	In the interim, for your request to be considered, all scheduled installment payments
	must be made as agreed.
